GUPPY
Isaac turns into his cat Guppy after picking up any 3 items from the list:
*Guppy’s Head – active item
*Guppy’s Paw – active item
*Dead Cat – passive item
*Guppy’s Collar – passive item
*Guppy’s Hair Ball – passive item
*Guppy’s Tail – passive item
As Guppy, Isaac gains flight and spawns a blue fly every time he hits an enemy with a tear.
Beelzebub
Isaac turns into giant fly, Beelzebub after picking up any 3 items from the list:
*Jar of Flies – active item
*???’s Only Friend – passive item
*BBF – passive item
*Best Bud – passive item
*Big Fan – passive item
*Distant Admiration – passive item
*Forever Alone – passive item
*Halo of Flies – passive item
*Hive Mind – passive item
*Infestation – passive item
*Skatole – passive item
*Smart Fly – passive item
*The Mulligan – passive item
* Friend Zone – passive item
* Lost Fly – passive item
* Obsessed Fan – passive item

In this form, most types of enemy flies will automatically be converted into blue flies.
Super Bum
Completing this transformation will take the THREE BUMS and replace them with the Super Bum familiar, who automatically collects all coins, red hearts, and keys in the room and drops twice as many rewards as the regular bums would have produced.

In this form, Isaac gains a stationary Mom’s Knife that trails behind Isaac as he moves, dealing 2x Isaac’s base damage on contact.
Bob
Isaac turns into Bob after picking up any 3 items from the list:

*Bob’s Brain – passive item
*Bob’s Curse – passive item
*Ipecac – passive item
Isaac gains a green trail of poisonous creep that deals 2 damage per tick or 6 damage per second.
Fun Guy
Isaac turns into Fun Guy after picking up any 3 items from the list:
*1up! – passive item
*Blue Cap – passive item
*Magic Mushroom – passive item
*Mini Mush – passive item
*Odd Mushroom (big) – passive item
*Odd Mushroom (small) – passive item
*God’s Flesh – passive item
Issac grants a heart container.
Leviathan
Isaac turns into Leviathan after picking up any 3 items from the list:
*The Nail – active item
*Abaddon – passive item
*Brimstone – passive item
*Pentagram – passive item
*Spirit of the Night – passive item
*The Mark – passive item
*The Pact – passive item
*Maw of the Void – passive item
(All the items are from devil room)
Issac gains 2 black hearts, and gains flight.
Conjoined
Isaac turns into Conjoined after picking up any 3 items from the list:
*Brother Bobby – passive item
*Harlequin Baby – passive item
*Headless Baby – passive item
*Little Steven – passive item
*Mongo Baby – passive item
*Rotten Baby – passive item
*Sister Maggy – passive item
Isaac grows two tumor faces from his cheeks, which fire simultaneously and diagonally similar to the effect of “The Wiz”. Decreases Isaac’s damage by 0.3 and slightly decreases Isaac’s tears.
Spun
Isaac turns into Spun after picking up any 3 items from the list:
*Experimental Treatment – passive item
*Growth Hormones – passive item
*Roid Rage – passive item
*Speed Ball – passive item
*Synthoil – passive item
*The Virus – passive item
*Adrenaline – passive item
*Euthanasia – passive item
Isaac gains +2 damage and +0.15 speed and 1 random pill.
Seraphim
Isaac turns into Seraphim after picking up any 3 items from the list:
*The Bible – active item
*Dead Dove – passive item
*Guardian Angel – passive item
*Holy Grail – passive item
*Holy Mantle – passive item
*Mitre – passive item
*Rosary – passive item
*The Halo – passive item
* Sworn Protector – passive item
Isaac will gain 3 soul hearts and permanent flight.
Oh Crap
Isaac turns into Oh Crap after picking up any 3 items from the list:
*Flush! – active item
*The Poop – active item
*E Coli – passive item
Isaac will heal half of a red heart each time a pile of poop is destroyed.
Bookworm
Isaac turns into Bookworm after picking up any 3 items from the list:

*Book of Revelations – active item

Isaac will fire tears out of both eyes simultaneously 25% of the time.
Spider Baby
Isaac turns into Spider Baby after picking up any 3 items from the list:
*Box of Spiders – active item
*Spider Butt – active item
*Mutant Spider – passive item
*Spider Baby – passive item
*Spider Bite – passive item
*Spider Mod – passive item
Isaac will gain a Spider familiar that applies random status effects to enemies it comes into contact with.
Adult
Isaac turns into Adult after taking 3 Pubert pills.
Isaac will turn into an adult and gain a heart container. Isaac’s voice will also become deeper.
Stompy
Upon getting three items or pills that increase Isaac’s size, Issac will create dust clouds when walking and have a chance to spawn rock waves around him upon taking damage.
Size up items:
